The Norwegian Healthcare Investigation Board, (NHIB) is an independent government agency. Our mandate is to investigate serious adverse events and other serious concerns involving the Norwegian healthcare services.

Our investigations focus on systems and processes in healthcare, identifying factors that could have led, or could potentially lead, to harm for patients. We do this without attributing blame or liability.

Our purpose is to improve patient and user safety by learning for improvement.

We decide for ourselves which serious adverse events or circumstances to investigate, the timing and scope of the investigations, and how this will be executed.

Our investigations are performed in dialogue with the parties involved, which include employees in health care services, patients/users of health care services and their families.

Our reports are public, however we make no references to the names and addresses of individuals involved. The location where the adverse event happened is as a general rule not disclosed, as we go into cases that could have happened everywhere in the Norwegian healthcare and our goal is national learning and improvement.

NHIB's activities are regulated by Act of 16 June 2017 no. 56 on the Norwegian Healthcare Investigation Board.
